How does the SurvivorRxPlan process work?
- Complete the application and mail or fax to SurvivorRxPlan. SurvivorRxPlan Application
- Our enrollment staff will review the application for eligibility requirements
- If approved, the application will be forwarded to our “Clinical” staff for review. Our clinical team will determine the most cost effective generic or therapeutic equivalent medication option.
- One of our clinical counselors will contact you by phone and discuss with you our suggestions that you can share with your physician.
- Get prescription from your physician; Your physician can call or fax us with the prescriptions, or you can mail us the new “hardcopy” prescriptions to: SurvivorRxPlan
- Please allow 3-5 business days for medication delivery, after we receive prescriptions.
***Please Note: Your physician is not required to give you a prescription based on our clinical staffs recommendations. Your physician is welcome to contact our clinical staff to discuss their suggestions.
